VOLUNTEER BACKGROUND CHECK GUIDE
Background checks are run at the Mentor Board of Education office Door 1
located at 6451 Center Street, Mentor, OH 44060 ~ no appointment is needed.
Office hours are Mon, Weds, Fri from 11:30am - 4:30pm.
****A Photo ID is required to run the background check.***
You will be asked to provide your Social Security number.
Minors must have a consent form signed by a parent or guardian.
Results will be emailed to you at the email address provided on the waiver.
Results may take up to 30 business days.
